As you’re preparing for your first visit or doing research on our practice, there are a few frequently asked questions our team receives on a regular basis that may help you understand more about what we do and how we can help you.
You are given an "arrival time" so the front desk can make sure all necessary paperwork is complete and your patient chart is ready prior to your appointment.
This depends on the type of insurance you have. Ohio does have Direct Access which allows patients to come without a physician referral if covered by your insurance carrier. Please call one of our offices for further information. We will also verify your insurance benefits prior to your first appointment.
Although a physician or other health care professional may suggest a certain facility or physical therapist based on experience or geographic location, it’s important to know you have the right to choose your physical therapist. You are not obligated to receive physical therapy in any specific facility.
We will assist you as much as possible in determining your insurance benefits, but there is no guarantee of benefits or payments per your insurance company disclosure. The best option is to call the number on the back of your insurance card and personally inquire about your benefits.
This is not a complete list of accepted insurance, so please call our office directly to see if your specific insurance plan is accepted. Some of the more common insurances accepted at Ohio Sports & Spine include Medicare, Medicare Advantage Plans, Ohio Medicaid and Medicaid HMO's, Aetna Commercial and Medicare, Anthem Commercial and Senior Advantage, CareSource Commercial, Medicare, Medicaid, Humana Commercial and Medicare, Medical Mutual Commercial and Medicare, United Healthcare Commercial, Medicare, and Medicaid, Workers’ Compensation, Motor Vehicle Accident
We accept all personal injury cases (Auto / Sports insurance medical policies): CareSource, Devoted Health, DOL, Molina, and WellCare.
We will verify your coverage and benefits for you prior to your first visit. We will inform you of the copay amount and explain your benefits at your first office visit. If you do not have health insurance or other coverage or have exhausted your insurance benefits, we do offer affordable payment plans. Please call our office for more specific details.
It’s best to wear loose, comfortable clothing and sneakers or comfortable shoes for physical therapy.
At your initial evaluation, your physical therapist will document the history pertaining to your injury or limitation, as well as perform a comprehensive examination to identify range of motion, flexibility, and strength. Your therapist may also provide hands-on services the day of, as well as stretches and exercises to complete at home prior to your next appointment.
Initial Evaluations and Treatment Sessions: You should allow at least 90 minutes for your initial evaluation and treatment session, including 30 minutes for paperwork, unless completed ahead of time. You can download the necessary forms here.
Your Plan of Care and Treatments will be coordinated and directed by one primary Physical Therapist. We utilize Physical Therapy Assistants, who may be part of your Plan of Care and utilized for your treatments and provide you with additional scheduling opportunities.
